چکیده

Tumefactive demyelinating lesions (TDLs) are often misdiagnosed as brain tumors. To identify imaging characteristics of TDLs for their proper diagnosis, their differences from malignant gliomas, etc., were investigated based on 3-Tesla magnetic resonance imaging (MRI) and MR spectroscopy data that was obtained from 5 patients diagnosed with TDLs in our hospital. MRI displayed a paraventricular lesion with a maximum diameter of ≥2 cm. Diffusion-weighted imaging (DWI) displayed a region of bright signal intensity at the margin of the lesion, and apparent diffusion coefficient (ADC) imaging displayed high signal intensity at the core of the lesion. The mass effect was mild relative to the size of the lesion, and the edema around the lesion was mild to moderate. Gadolinium-enhanced MRI displayed a characteristic open-ring sign, in which the ring of enhancement was incomplete (open towards the cortex), with the medullary vein running through the lesion, resulting in the enhancement of the surrounding area. MR spectroscopy displayed an increase in the choline peak, a decrease in the N-acetyl aspartate (NAA) peak, the presence of lactate/lipid peaks, and the presence of β, γ-glutamate-glutamine (GLX) peaks that are slightly shifted to a higher resonance frequency (2.1 2.4 ppm) from the NAA peak. Taken together, the following main characteristics were identified as imaging features of TDLs: bright rim on DWI, a high signal core on ADC imaging, an open-ring sign, and β, γ-GLX peaks on proton MR spectroscopy. We concluded that the key to the improvement of neurological symptoms of patients is the prompt initiation of steroid treatment following the accurate diagnosis of TDLs.

Tumefactive Brain Lesions in Patients with Neuromyelitis Optica Spectrum Disorder

Background: Neuromyelitis Optica Spectrum Disorder (NMOSD) is an autoimmune neurological disorder that is characterized by optic neuritis and longitudinally-extended transverse myelitis lesions in spinal segments. Magnetic Resonance Imaging (MRI) findings are part of the diagnostic process in NMOSD patients, and abnormal lesion patterns may cause deviation from a correct diagnosis.  Clinical P...

Tumefactive: A Rare First presentation of Multiple Sclerosis

Tumefactive demyelinating lesions are a rare presentation of multiple sclerosis (MS). Diagnosis of tumefactive is commonly carried out using magnetic resonance image (MRI). Tumefactive diagnosis is difficult because of may similar to the clinical and MRI characteristics of glioma or a cerebral abscess. We presented a 35-years-old female with one episode of secondary generalized seizure after de...

Differentiation of Tumefactive Demyelinating Lesions from Metastatic Brain Disease with FDG PET-CT: A Case Report

Tumefactive demyelinating brain disease can pose a diagnostic challenge in patients without a pre-existing diagnosis of multiple sclerosis by mimicking intra-cranial neoplasms, infarction, as well as infections. Choosing when to biopsy a tumefactive lesion to exclude alternative pathology can be difficult. Here we report a case of a 44 year old female patient without a previous history of multi...

Tumefactive Multiple Sclerosis Variants: Report of Two Cases of Schilder and Balo Diseases

A tumefactive lesion of central nervous system (CNS) is defined as a mass-like lesion with a size greater than 2 cm in brain detected by magnetic resonance imaging (MRI). Neuroimaging may help to distinguish the nature of a tumefactive lesion and therefore, can prevent an unnecessary brain biopsy. Here we emphasized on determining the nature of a CNS tumefactive lesions with the help of MRI and...

Metabolite findings in tumefactive demyelinating lesions utilizing short echo time proton magnetic resonance spectroscopy.

BACKGROUND AND PURPOSE To use MR spectroscopy to aid in the diagnosis of demyelinating disease and to help differentiate tumefactive demyelinating lesions from neoplastic processes. MATERIALS AND METHODS MR imaging of the brain was obtained in 4 patients who presented clinically with focal neurologic deficits. MR imaging initially revealed parenchymal mass lesions. Single-voxel MR spectroscop...

Mass lesions in the brain: tumor or multiple sclerosis? Clinical and imaging characteristics and course from a single reference center.

AIM In demyelinating disease spectrum, tumor-like (tumefactive) demyelinating lesions (TDL) are rarely seen. Atypical imaging and clinical features of these lesions may cause misdiagnosis of tumor or abscess. MATERIAL AND METHODS 25 patients with TDL in our center were followed and clinical, magnetic resonance imaging (MRI), magnetic resonance spectroscopy, cerebrospinal fluid (CSF) findings ...
